7/3/2023 EnchantaVaughn Approved 1.Behavior: JaCarei displayed aggressive responses towards his sister and attempted to hit her in the face. JaCarei participated in verbal altercation with peers at school and started to utilize cursing and threatening comments. 2. Behavior: JaCarei presented hostile behaviors as he was ignoring QMHP-C at times within the session and when redirected, he would not listen and turn away. 1.Intervention: QMHP-C discussed JaCarei mood and provided role playing activities to help JaCarei with demonstrating positive facial expressions and positive communication. QMHP-C provided video that demonstrate anger outburst and utilize deep breathing, counting to ten, and walking away. QMHP-C encouraged JaCarei to implement what has been taught to him during sessions on a daily basis to prevent him from getting in trouble with his mother. 2. Intervention: QMHP-C assisted JaCarei with making a stress ball to help decrease his anger by squeezing with feeling upset. QMHP-C coached JaCarei to utilize anger management techniques to ensure JaCarei making positive choices and ignoring negative situation. QMHP-C encouraged JaCarei to apply all the skills taught to him by his QMHP-C everyday to prevent from getting in trouble. 1.Response: JaCarei responded appropriately as evidenced by JaCarei looking at the video and understanding the question provided. JaCarei was appropriate by stating that he attempt to walk away and seems to get influenced by other peers. 2. Response: JaCarei was receptive by indicating that he argues with his siblings at least3 to 4 times a week. JaCarei reported he continues to try and do what he has to but people make him upset.
